- Degree in Pharmacy or equivalent
- At least 5 years of relevant work experience
- Deliver pharmacy services to DHC clients at core and satellite centers, including clinical and community pharmaceutical care, such as drug counseling, medication compliance education, and promoting medication literacy.
- Design, develop, and implement educational/training programs on medication management and safety.
- Perform medication reviews and provide counseling.
- Provide smoking cessation counseling.
- Engage in multidisciplinary case conferences.
- Registered with the Pharmacy and Poisons Board of Hong Kong, holding a valid practicing certificate.
- Degree in Pharmacy or equivalent.
- Minimum 5 years of relevant experience in health and social services.
- Mature, dedicated, problem-solving, and collaborative team player.
- Strong communication and interpersonal skills.
- Proficient in spoken and written English and Chinese.
