Social Worker

• Develop and manage the counselling services plan, including new case connection, patient outreach and hotline; and perform follow-ups assessment

• Provide counselling to children / adolescent with diabetes and their parents regarding issues including mental health, child care, and medical care

• Maintain detailed case records, input member and case data into internal systems, and prepare monthly service reports

• Serve as liaisons between hospitals, medical professionals, patient groups and schools and other stakeholders

• Design, lead, manage and promote member programmes such as mentorship, workshops, family events and children / adolescent activities

• Prepare event budgets, cost comparisons and post-event reviews

• Achieve deliverables of funded programmes and compile reports for management

• Ensure consistency of quality, accountability and high standards in all programmes and services

• Registered social worker

• Preferably 1 year or above relevant working experience

• Proficient in written and spoken English and Chinese

• Strong passion for working with youth and children

• Responsible, self-motivated and attentive to details

• Willing to work in non-office hours or weekends for activities and events (compensation leave will be provided)

• Period : 2 years (renewable)

• Work Schedule : 5-day work week
