Care Coordinator (Contract) (Shum Shui Po District Health Centre)

• Bachelor of Nursing

• Part I or II Registration with the Nursing Council of Hong Kong and with valid practicing certificate

• Registered Nurse with at least 5 years post-registration clinical experience

• At least 2 years experiences in supervisory role

• Complete Post-registration Certificate Course in Community Nursing/Primary Health Care / Public Health are preferable

• Strong leadership with good decision making and analytic skills

• Able to work independently with good problem-solving skills and flexibility

• Coordinate, implement, monitor and evaluate various primary healthcare services

• Lead the team in carrying out health management programmes, intervention groups and workshops to service users with healthcare needs, involving case management and outreach activities

• Supervise and coach team members to ensure productivity, services quality and continuous improvement

• Assist in developing various healthcare services and service strategies

• Work closely with different stakeholders and multi-disciplinary teams to deliver primary health care services

• Any ad hoc duties assigned by supervisor

• Shift duties are required
